Memory Care

Memory Care can be found in Assisted Living communities as well as smaller residential settings, such as Adult Family Homes. Memory Care focuses on residents with Alzheimer’s and other forms of Dementia. Each community is trained and specializes in providing resident specific care. The community is set up to provide safety, enhance memory functions and to decrease potential confusion for residents. Residents who are appropriate for this setting need an increase in assistance with activities of daily living as they are forgetting how to perform daily activities, frequently getting lost, wandering or going into other people’s room. The structured and supportive environment provides dignity, allows opportunities for involvement and empowers each resident to participate in hands on activity without stress and duress. Due to the increase in quality and quantity of staffing the price for Memory Care is greater than Assisted Living communities.

Skilled Nursing Facility/ Rehab Center

Skilled Nursing Facilities (SNF) or Rehab centers facilitate recovery from acute care incidents and accidents or major health issues. Once discharged from the hospital, care may be required for an additional period of time, SNFs are able to provide these additional therapies on site. Close supervision or services from a physician, nurse, social worker, physical therapist, occupational therapist or a respiratory therapist may be required. Insurances such as L & I, LTC Insurance Medicaid Insurance are used as a form of payment if an individual qualifies. Most seniors will never need a nursing home due to the increased services that assisted living and Dementia/Alzheimer’s/Memory Care Communities can perform.

Hospice Care/End of Life Care

Hospice and End of Life Care provides supportive care to people in the final phase of a terminal illness and focuses on comfort and quality of life, rather than finding a cure. A patient on Hospice is expected to live 6 months or less if the illness runs its usual course.  The ultimate goal is to provide comfort and limit pain, so that each day lived as comfortable as possible. The service can be provided at Home, Assistant Living, Independent Living or at an Adult Family Home. The cost is dependent on the care needs required.

What Happens if Care Needs Change?

If care needs change and your loved one’s health declines, the staff of the senior living community will complete a new Nursing assessment and Care Plan. By doing this it ensures that the staff is able to appropriately meet the new needs of the resident, and that the resident’s safety and well-being are considered. Changes in care plans typically relate to an increase in care expenses. If the senior living community feels they cannot meet the needs of the resident, they will make a referral back to Sterling Senior Advisory to assist with alternate options for housing and care.
